Leading dementia education for his team, Jerry has built a proactive, empathetic team culture over the last year. When a resident experiences an on-the-floor event, Jerry encourages the team to get down to their level. By the time fall huddles begin, Jerry has often already contacted families, looped in health care providers, and put physical interventions in place.

His nominator, Sara Padilla, Vice President of Health Services, cited exactly that combination of knowledge, leadership, and humility in putting his name forward.
